Our Extended Care Services

• Home Health Care: For patients of all ages, Rochester Regional offers in-home care to assist with everything from post-surgical care to disease management.

  In-home services include:

  • skilled nursing
  • rehabilitation therapies and care
  • medical social work
  • home infusion therapy
  • home health aides
  • cardiopulmonary care
  • diabetes care
  • cancer care
  • Parkinson’s care
  • wound care
  • joint replacement care 

• Skilled Nursing Homes: Rochester Regional offers several skilled nursing homes, each providing its own specialty of care, including: dementia and Alzheimer's disease, short-term rehabilitation, transitional care, ventilator units and neurobehavioral care. Staff members provide plenty of personal attention and ensure that patients are treated with highest level of medical care and skilled services.

• ElderONE: The ElderONE program takes a collaborative approach to caring for seniors. It combines medical, transportation and personalized care services so older patients can live at home longer and maintain their independence without compromising their medical care or safety. The ElderONE care team assesses the needs of every patient and – with input from patients and their loved ones – develops an individualized care plan. As care needs evolve, the care plan is adjusted. ElderONE provides services both in the home and in a day center setting.

• Adult Day Programs: Rochester Regional Health offers the full range of adult day services for people with all levels of needs. The medical adult day care programs offer comprehensive medical, social, and rehabilitation services, in addition to therapeutic recreational activities. Meanwhile, the social adult day care program sites feature large activity rooms, outside patios, a special safety and security system and engaging programs that offer both group activities and quiet time.

• Hospice Care: For those in need of hospice care, Rochester Regional Health can support people in their own homes, in volunteer-run comfort care homes, in skilled nursing homes and at the Elizabeth G. and Jennifer J. Hildebrandt Hospice Care Center. Our hospice care gives patients and families the chance to spend time together with comfort and dignity, while receiving services focused on sustaining quality of life.
